1. Do not use if the product, protective packaging, or sterile packaging is damaged, opened unintentionally before use, and/or exposed to environmental conditions outside of those specified in the IFU.

2. This device was designed and tested for single patient use only. Do not reuse, reprocess or resterilize this device. Reuse, reprocessing or resterilization may alter the structural and/or functional integrity of this device which may result in patient injury, infection, illness or death. The manufacturer will not be responsible for any direct, incidental or consequential damages resulting from resterilization or reuse.

3. Avoid sharp instruments contacting the device, as it may damage the device or cause patient injury.

4. Please carefully read all instructions prior to use. Failure to observe all warnings and precautions may result in complications.

5. After use, the product may be a potential biohazard. Handle and dispose of in accordance with acceptable medical practice and applicable local, state, and federal laws and regulations related to product contamination by blood.
